46.2% of Black Hawk County residents 18 years and older have received a COVID-19 vaccine as of April 12,2021.

Moderna(ages 18+) or Pfizer (ages 16+) vaccines are currently available in Black Hawk County. Please refer to the Black Hawk County Public Health website for updated information regarding the Johnson and Johnson vaccine.

Identification cards or insurance information are not required, however all registrants will need to provide contact information and complete a consent form prior to vaccination. Please note that your second dose will be automatically scheduled for the same time 21 days later (May 15 or 16, 2021). If you cannot commit to the second appointment, please consider another vaccination option.
