Child and Adolescent Health Services

The Public Health Center offers these no or low-cost services:

·      Child and Adolescent Health Exams (Well-Child Visits) - Ages 0-20

·      Immunizations through the Vaccines for Children Program - Ages 0-18

·      Preventive Dental Screenings - Ages 0-20

·      Blood Lead Testing - Ages 1-6

·      Sports Physicals - $20 fee

Hours and Scheduling

Annual Health Exams, vaccines, blood lead testing, and sports physicals:

Monday: 9:30 - 11:30 a.m. & 1 - 4 p.m.
Tuesday – Friday: 8:30 – 11:30 a.m. & 1 – 4 p.m.

Please note: The clinic will be closed the first Thursday of the month from 2 - 4 p.m.

These services are by appointment only. Call 319-291-2413 to schedule an appointment.

Preventive dental services:

Mondays: 9:30-11:30 a.m. and 1-4 p.m.

Annual Health Exams (Well Child Visits)

Eligibility:

If your child does not have a medical provider and either does not have insurance (no insurance )or is underinsured (insurance does not cover services), we offer no‑cost or low‑cost yearly health exams for ages 0–20.

 

Services include:


• Comprehensive health and development assessment
• Hearing and vision screening
• Physical exam
• Review of immunizations

Vaccines for Children (VFC) Program

Eligibility:

Children 0-18 who are uninsured (no insurance), underinsured (insurance does not cover vaccines), Medicaid-enrolled*, or American Indian or Alaskan Native*.

*Some healthcare providers also offer the Vaccines for Children program. If your child has a provider, contact their office for information on vaccines available.

DT, DTaP, Hepatitis A, Hepatitis B, Hib, HPV, Influenza, Menactra, Meningococcal B, Polio, Pneumonia, Rotavirus, Tdap, Tetanus, Varicella

Blood Lead Testing

Eligibility:

Children ages 1–6 who are due for lead testing

Preventive Dental Care

Eligibility:

If your child doesn't have a dentist, our Child Health Clinic offers no-cost preventive dental care for children and adolescents ages 0–20.

Available services:

• Oral screenings

• Fluoride varnish

Insurance Navigation Services Available

Children and families needing insurance coverage will be referred to the BHCHD Hawk-I Coordinator who will assist them in applying for Presumptive Medicaid or Hawk-I insurance.
