The Public Health Center offers child /adolescent well-child exams, lead poisoning prevention screening, and childhood immunizations by appointment only.

Children ages 2 months and older

What to bring

Up-to-date immunization record.

Insurance information. 

Children under 18 must be accompanied by an adult.

Insurance Navigation Services Available

Children and families needing insurance coverage will be referred to the BHCHD Hawk-I Coordinator who will assist them in applying for Presumptive Medicaid or Hawk-I insurance.
